GUEST COVID-19 QUESTIONNAIRE

HOW DO WE PROCESS INFORMATION COLLECTED VIA THE QUESTIONNAIRE?
We collect and process the following information through our vendor for the purposes set forth below.
Health Status and COVID-19 Information. The Questionnaire will ask you for your name, email address, and phone number; your household members’ names, if applicable; information about your, and your household members’, current health status, symptoms, COVID-19 testing status and results; and your contact with people who have or may have COVID-19. We may also receive information about your language preference, and you may share information via text message for support with our vendor.
We use this information to determine entry into the facility, to provide your screening pass via text message, to help ensure the health and safety of our employees and patrons, to monitor and respond to instances of COVID-19 in our facility, to make decisions relating to closures or disinfecting the facility, to enforce our policies, to communicate with you about your questions, and to manage our business operations.

Temporary storage of previous URL, current URL, whether or not the user is logged in, error messages, and success messages
Stores an encrypted timestamp unique to the device being used to check in and is used to verify return users
Stores the encrypted phone number originally submitted to check-in by that device to help ensure the device viewing a confirmation link is the same device that submitted the form
We process this Technical Information to analyze use of the Questionnaire and to operate and ensure the privacy and security of the Questionnaire.
Other Processing Activities. We may also process the information described above when necessary for: the establishment, exercise, or defense of legal claims, whether in court, administrative, or other proceedings; and obtaining or maintaining insurance coverage, managing risks, or obtaining professional advice.
How do we share the information described in this Notice? Where permitted, information collected via the Questionnaire may be shared in the following contexts:

We may share information with our corporate subsidiaries and affiliates and with their respective officers, directors, employees, and agents.
Legal Obligations and Rights
We may disclose information in response to subpoenas, warrants, court orders or other legal process, or to comply with relevant laws. We may also share your information in order to establish or exercise our legal rights; to defend against a legal claim; and to investigate, prevent, or take action regarding possible illegal activities, suspected fraud, safety of person or property, or a violation of policies and agreements.
Service Providers and Professional Advisors
We may share information with our service providers that need access to the information to provide operational or other support services on our behalf. Among other things, service providers help administer the Questionnaire, support our operations, assist with insurance, provide legal or other professional advice, and assist with other legitimate purposes permitted by law.
OTHER IMPORTANT INFORMATION
How long do we keep your information? We will store your information for 20 days from your last visit. If you return within 20 days, we will associate your current check-in with your name and email address from the earlier check-in and keep the new data for the next 20 days. Your earlier check-in results, texts, and chats will be erased once older than 20 days. Cookies will be stored on your device for the amount of time described above. How do we protect your information? We have put reasonable security measures in place to protect the information that you share with us from being accidentally lost, used, altered, disclosed, or accessed in an unauthorized manner. Our vendor also maintains security measures, such as encryption, firewalls, and intrusion detection and prevention systems, to protect sensitive information. However, because no transmission over the internet is 100% secure, we cannot guarantee that your information will remain absolutely secure in all circumstances.
Updates and Changes. We may add to, change, update, or modify this Notice to reflect any changes to how we treat your information or in response to changes in the law. Should this Notice change, we will post all changes to this Notice on this page.
